Reading Health System is seeking additional Hospitalists for our Rehabilitation Hospital.

Our Hospitalist opportunity offers a unique practice which combines managing a wide range of medical and surgery sub-acute patients during their transition to home. The schedule is day shift and consists of two weeks on, one week off. With the additional hospitalists, the minimal night call from home will be 1:6. We are seeking physicians who are Board Certified in Internal Medicine.

Responsibilities include coordinating all aspects of care for the TSU (Transitional SubActue Unit) patients and provide co-management services for TRC (Transitional Rehabilitation Center) patients:
* Admit and manage the clinical care of Adult Medicine Service patients on a sub-acute unit.
* Discussion of End of Life preferences.
* Comprehensive medical consults with sub-specialists on acute rehab patients.
* Coordination of care with primary care physician.
* Discharge planning, including ensuring appropriate follow-up is arranged.
* Coordination/co-management of care with physiatrist on all of their patients.
* Participate in quality improvement activities.
* Manage patients in a high-quality, cost effective fashion.
* Inpatient attending for Internal Medicine Residents.

* 50-bed Transitional Care Center for skilled care

* On-site pharmacy, laboratory and radiological services
